The Home and Community Care Support Services (HCCS) coordinates and controls admissions to Ina Grafton Gage Home. To be considered for admission, you or a family member on your behalf, must complete a Placement Application Form, which is available from the HCCS. The HCCS will provide a case manager to facilitate the placement process.

Due to the high volume of applicants, there is a waiting list to enter our home. We suggest getting the name of your loved one in the queue as soon as you feel that they will require long term care. Once a bed becomes available, you will be contacted by the HCCS with an offer of admission. This offer must be accepted within 24 hours and if accepted, admission to Ina Grafton Gage Home must occur within five days.

The Ministry of Health and Long-Term Care pays for the care you receive, but you must pay for the accommodation costs, as shown below. The accommodation rates are adjusted periodically by the Ministry and are standard across Ontario.



Residents with reduced rates ending on June 30, 2022, are required to reapply for a new rate reduction.
